  This *should* work:

    - Control Center -> Security & Privacy -> Privacy -> Web browsing ->
      Form completion entries SELECTED -> Cleanup

  *If* it doesn't, then do:
  
    $ rm ~/.kde/share/apps/khtml/formcompletions
