amavisd-new not rewriting headers

Postfix 2.3 on Sarge. Current amavisd-new and spamassassin.
All current versions from upstream.
Postfix smtpd proxies incoming SMTP to amavisd, amavisd
adds its Received: headerfield and passes the message
to another smtpd that doesn't proxy. Postfix queues
and delivers the mail. This is the amavis before-queue
setup. Loadaverage on the machine is 0.12.

Everything else at default settings. Mail goes through
and gets logged. But no X-Spam headers are ever added
to the messages. How can I troubleshoot this?
The sample config files are really great but there is
not much about what if it doesn't work.
