Here's the id numbers on my qla2200 card:

Stamped on card solder side of card:
PWB FC0210706-00-A(REV)

Bar coded labels on comp side of card:
FC0210406-13 C
TFC0226G21619 QLA2200/66

Bar coded label on solder side of card:
13550-27V-E029 DP/N 0001280R Rev. A00

Hope one of these are what you were asking for. If not, please let me know and I'll correct.

I notice the qla2x00-source debian package in debian/unstable:

  Package: qla2x00-source

  Description: Source for the qla2x00 driver for kernel version 2.4
   This package provides the source code for the qla2x00 kernel

Perhaps it can help?  If you can provide me with PCI IDs of the cards
in question, I can teach discover-pkginstall from the discover package
to install it automatically on the machines in question.  I prefer if
such info is submitted into the debian bts to make sure I do not miss
