Bug#608022: make it clear that only upgrades from lenny are supported

It's not been policy to require that all point releases have been
applied before updating, although we do encourage updates in the release
announcement for each point release.  Theoretically one should be able
to upgrade from anywhere between the original release and the latest
point release + p-u.  In practice we know that this won't work for some
packages as upgrade issues have only been fixed in later point releases.

Unless the packages affected are part of a standard or base system, I'm
not sure we want to start insisting that people fully update / upgrade
their stableN system before beginning to upgrade to stableN+1.
