release notes: How to upgrade from kernel 2.4 to 2.6

Hello,

The upgrade reports show that users tend to upgrade the kernel from 2.4
to 2.6 at the same time they upgrade to sarge, so it could be worthwhile
to include a note to help them:

If you are upgrading from a 2.4 to a 2.6 kernel, before rebooting:

1) You must install modules-init-tools.

2) If you have an LVM1 partition, you must install the 'lvm2' package.

Also I think you should remove section 4.1.4:

- 4.1.4 Special considerations for SSH users this change was made in
  woody, not sarge.
