That's already being done by the CVS server (see for instance
http://cvs.debian.org/ddp/manuals.sgml/faq/software.sgml?cvsroot=debian-doc)
You can extract that info into a Changelog file using cvs2cl, or include it
into a comment in the document itself by inserting a $Log:$ keyword.
