I have fixed it in upcoming http://www.debian.org/doc/cvs.html, to point to
aph (as Adam is the DDP coordinator now, and the wording seemed to imply the
coordinator is the one who creates accounts).

I can fix the old document, too, but it should be phased out anyway.
