Not many things. I'm aware of one package, but that's a bug in it, I just can't find enough time to patch it. In general Tcl/Tk upstream is very careful about changes, and it's very rare that compatibility breaks. Even ABI is kept very stable so usually you can use binary extensions from old Tcl versions with new ones.
