AFAIR, you only have to do some additions in /etc/kde3/kdm/Xservers.
Here's mine:

you need to restart kdm.
A new entry should appear in the K-Menu: 'start new session' (translated from 
german...)
And the unlock-dialog of a (kde)screensaver should provide this possibility 
too
