I'd guess a hardware problem. Many years ago, I experienced something similar: memory checked okay, disk checked okay, but still occasional corruption (avg. 1 bit per week or so). Memory changed, disk changed, still corruption. Finally found out that the northbridge was just a trifle unstable at the auto-selected frequency. Underclocking the memory bus and increasing the multiplier fixed it, and today that system is still running just fine.
