This turned out to be the use of pxelinux.0 from the Lenny beta2 with the
vesamenu.cfg from the dailies.  Once I upgraded to the pxelinus.0 from the
dailies the problem went away.  This mail is just to document the solution
in case someone else makes the same (stupid) mistake.
