If I'm reading this correctly, your device is pci id 1de1:0391, for
which we currently load the dc3x5 module:

pci.lst:        1de10391        scsi    dc3x5   TRM-S1040

If that module doesn't work and the dc395 is needed instead a bug should
be split out to discover1-data along those lines.  FWIW, modules.pcimap
also lists 1de10391 as the module id for the dc395:
