That is decidedly weird.
The main thing to consider is that RAID/LVM support in the installer is 
relatively new. It will probably be improved after the next release.

No idea. Debugging mainly means running relevant scripts with debugging 
turned on or actually debugging compiled programs until you stumble onto 
something that looks broken...
Running a setup like 4- in qemu could maybe help.

I think the best thing is to file an installation-report [1] with the info 
you've send so far so that we can keep track of this issue.
Suggest you file this against debian-installer rather than 
installation-reports. However, our priority is on releasing ATM, not 
bugfixing.
