Bug#235365: "cancel this menu" redundant with most frontends; redesign

In partman, if I choose a device, the first menu item is "Cancel this
menu", which is redundant because there is a back button. The third item
is something about dumping the partition info, which might be useful for
debugging, but I did not understand. The second item is the only one I
think anyone will want to select.

In discussion on the mailing list, we decided that the debugging thing
should go in bugreporter-udeb somehow. Then that whole little menu can
be changed into a boolean question that asks the user if they want to
create a new, empty partition table on the selected disk. It should
default to false.
