create many partitions with pressed

HI,

i would like to build my own Denian OS bootable and auto with the simple-cdd
application.
i don't have experience about this process to create partitions with
partman-auto and recipe.

my hard disk has a size of 500Mb and I want to have partitions like that:
partition "/" equal to 150 GB
partition "swap" equal to 12GB
partitions "/tmp , /home, /usr, /srv," equal to 10 GB
partition "/var" equal to 150 GB

I want to understand how to make patitions with preseed/recipe
