Why don't you try MILO and see if your machine works better that way? I
have a 166MHz AXPpci33 and I have had no problems with MILO 2.0.29 from
ftp://gatekeeper.dec.com/pub/Digital/Linux-Alpha/ . I suggest that you
set the jumper to load MILO from floppy on power-up and then run the
fmu.gz FLASH update utility. Instructions for doing this can be found at 
the Linux/Alpha Home Page (http://www.azstarnet.com/~axplinux). You can
always reload SRM if this doesn't help.

Also the S3 ViRGE X-server is known to be buggy, so try doing some
testing in text mode. I have a 4MB ViRGE card and have had no problems
like the ones you have described, but I am running in text mode only.
