Common usecase? 
It should be possible to use a console screenreader with braille and
speech running on the same machine with orca.

I.ll try to describe my setup to avoid problems with pa:
1. in default.pa add the param device=dmix
so pulseaudio can't open audio device with hw0,0 which would block all
alsa output of other apps

2. set SPEECHD_ADDRESS variable and reconfigure speech-dispatcher to run
in system mode.
So orca or an other screenreader can use it at the same time.

3. Switch audiooutputmethod in speechd to libao and use libao'Äs alsa
output backend.

This works for me without any problems.
