I've been using Limesurvey (https://www.limesurvey.org/). I can host an instance for DC16. IIRC Laura has it running as well. We should collect questions we would like to have a poll on. That way we can create one combined poll and bother people only once. This gets us more attention and statistically better results. I've created https://wiki.debconf.org/wiki/DebConf16/AttendeePoll so we can collect the questions in one place and not forget about them.
