This is not a good sign for that particular
media.

The percent number stems from a progress report
of the drive which is traditionally not very
trustworthy. So the 60+ % are not necessarily a
reason to worry.

Two minutes is a bit short for a full format.
Normally it should last the time needed for a
write run that fills the whole media.

>From reading dvd+rw-format.cpp i assume you
should try
  dvd+rw-format -force=full /dev/dvd
