Use the "--append-to-version" option in make-kpkg.
"--append-to-version foo" should use the directory
/lib/modules/2.4.18-foo/ for the modules.  (Not quite what you wanted,
but should be good enough.)  Make sure you read the entry in the man
page for that option.
