Part of the problem here is that the changelog isn't a particularly good
place to document the source of the package - it's roughly similar to
the situation with normal source code where you'd expect the code to be
readable without the changelog to explain it.

This would work adequately for trivial patches but is likely to have
issues if the patch is more involved and needs revisions (for things
like upstream changes or as part of making it mergeable).
