I'm CCing this to debian-devel to see if anyone else knows what could
cause this.
The general problem is that for this user, xexec is not properly adding
in the menu entry.

This is odd. It certainly shows up in olvwm, which is the WM used
on the compilation system. In the source package, the debian/menu file
is still present in the most recent version and has the contents:
?package(splay):needs=X11 section=XShells\
title="xexec" command="/usr/X11R6/bin/xexec"
Does your system have a Debian menu at all? A Debian/XShells? Does
it contain other entries such as Rxvt or Xterm?
