IIRC this has happened before with kernel/modules versions mismatching.
With USB and netboot you must boot with the same kernel you're going to use.
What files did you used to boot?

Ok, it seems I was using netboot files from May 2011. So I downloaded the latest ones from http://ftp.nl.debian.org/debian/dists/squeeze/main/installer-i386/current/images/netboot/
And now the error is slightly different, but still the same stalling at 59%:

device-mapper: uevent: version 1.0.3
