Bug#659986: installation-reports: Netboot fails with mdadm error at partition manager

Package: installation-reports
Severity: important
Tags: d-i

The Debian Squeeze netboot always fails with the error listed below,
when the "Starting Partition Manager" window has its progress bar at
exactly 59%.

kernel: [ 517.897823] Btrfs loaded
kernel: [ 517.947384] JFS: nTxBlock = 8021, nTxLock = 64169
kernel: [ 517.972474] SGI XFS with ACLs, security attributes, realtime,
large block/inode numbers, no debug enabled
kernel: [ 517.973530] SGI XFS Quota Management subsystem
md-devices: mdadm: No arrays found in config file or automatically
kernel: [ 518.263108] dm_mod: Unknown symbol scsi_verify_blk_ctl

I have been using netboot to install all my Debian boxes, and this
problem cropped up just a couple of days ago.

System info relates to the machine I'm doing the reporting from, not the
affected machine, as the installation is not successful there.
