Can you complement report with dvd+rw-mediainfo output for busted media?

Even though it's very unlikely to be a media problem, there still is a
chance that unit simply crashes trying to record on poorly supported
media. Do try another media brand, preferably DVD+RW (you should have
received one with your unit), so that you don't just waste blanks. If it
still fails, you have no other choice but to test it with another,
preferably 2.4.x, kernel, try to burn with DMA off, etc.

I also double-checked now and it turned out that I received similar
problem report: same unit, same LBA, approximately same time to failure,
but it was sane KEY/ASC/ASCQ values, not F/FF/FF (which signals *fatal*
condition)... Problem was caused by the fact that user applied "variable
HZ" kernel patch along with DVD+RW kernel patch, both 2.4 patches, which
screwed up command timeouts and command was prematurely terminated...
But, there shouldn't be problem with 2.6, at least patch addressing
similar timeout problem was incorportated prior first 2.6 cut... and the
code is still there... A.
