Using the standard Debian tool "dh_make" for creating a source package would have hinted you to have "Closes: #1015834" in your changelog entry. Also, we do not care which tool you used to do that as long as it is correct. So please replace the line with "Initial release (Closes: #1015834)". What is that adapter-latest.js file in debian/copyright? Please separate the Copyright info from the license text in debian/copyright.
