please change the changelog:

the bugs are not fixed by Debian packaging, but from upstream new release.

I would change it that way
+  * New upstream release
+   - Fix Multiplication of IPv6 addresses for upstream (Closes: #791582)
+   - Fix dhcpcd doesn't configure chrony for upstream (Closes: #791583)


did you forward the two patches upstream?

the other stuff looks good (note: I didn't try to build and run it)
