Bug reporting form

I have added a bug reporting form the LSB test tools. This will
allow the test team to collect all of the data they need to
analyze a failure and also eventually to populate a problem
report database.
The front end is a web form at
	http://www.linuxbase.org/test/lsb_bugreport.html

The backend message generated out of that is a flat text file
for now that is emailed to the lsb-test@linuxbase.org team.
We are anticipating revising the tools quite rapidly as needed
over the next 4-6 weeks in order to have a bug shakeout.
