I don't know what is "Application Form" in the following sentence.
www.linuxbase.org/test/pilot says*
"The following documentation should be submitted to the LSB Pilot team with an application for the LSB Build Environment Certification pilot:
LSB Certification Pilot Application Form, (this details the name and company of the applicant, a description of the distribution name and version, architecture, and signed agreement to the terms and conditions of the Pilot program)"
