No, this makes alot of sense for a number of reasons. First, the list of
packages to upgrade is often huge, there is no point in showing it because
you'd never be able to get anything usefull out of it.

Packages held back is important because most people use APT to keep their
system uptodate, if packages don't install via upgrade they need to know
that they haven't been upgraded so they can correct it.

In general upgrading is not a problem, removing and installing new can be
problems and that is why they are noted.

And finally, if you want a package to be not upgraded ever then put a hold
on the package. If that package is installed for any reason APT will
specially list it and warn you that it is being changed.

If you want to see the list use -s.
