I barely use xterm and I am afraid I did not ssh from an xterm.

No.  There isn't anything like that.

It happens from quite a few machines but I am able to logout sometimes
from a machine and sometimes I am not.  I am using the latest
xfce4-terminal and still see this problem.  The next time around, I will
try and strace on both the terminal and ssh and see where they are.
