No, it's not what I'm describing.

When I start xterm, I get black text by default, a black text cursor,
and a black I-shapped mouse pointer.

When I start xterm -fg red, I get red text, a red text cursor, and a
red I-shapped mouse pointer.

And switching the pointer from the first xterm to the second one, and
the other way round, makes it change colour each time.

Perhaps the problem is in the X server (specific to the video driver, for instance). The checks I've made were from different systems using the
same hardware.  fwiw, I've got this in my config:

Section "Device"
    Identifier  "S3 ProSavageDDR"
    Driver      "savage"
