Bug#527083: xserver-xorg-input-evdev: Loses xmodmap mappings

I don't know if this is the right package to file against, but it
seems the most likely.  When I start X I run xmodmap to do a bit
of setup:

remove Lock = Caps_Lock
keysym Caps_Lock = Control_L
add Lock = Caps_Lock
add Control = Control_L
keysym Super_R = XF86Back
keysym Menu = XF86Forward

(Maps caps lock to control, sets the silly windows keys to
go forwards and backwards in Firefox)
Since a recent dist-upgrade (I'm not sure exactly which - I
dist-upgrade regularly but often don't restart X), I'm finding
that I frequently lost these mappings.  Additionally, when I try
to rerun xmodmap to restore them, I sometimes get some unexpected
error messages (maybe unexpected just because I don't really
understand the X keyboard model):

Rerunning xmodmap sometimes works, sometimes doesn't, and sometimes
leaves the keyboard in a bit of a funny state.  For instance,
currently, my caps lock key is correctly functioning as control,
but it still toggles the caps lock light on my keyboard.

This all may be due to the fact that my (USB) keyboard seems to
detach and reconnect sometimes, but I don't think that should be
relevant - I can unplug and replug a PS/2 keyboard without
resetting key mappings.

Any hints about what my be going on would be appreciated, but the
current behavior seems wrong - I've been running essentially the
same X setup for over a decade and haven't seen this sort of
behavior before.
