Unless you need zaphod and xinerama for something, I'd suggest
removing the second device section and just use xrandr to configure
dualhead. zaphod mode only supports one output per driver instance
and in your case LVDS and VGA are getting selected. If you want to be
able to use LVDS, VGA, and DVI, you'll need to use xrandr. Also, your
DVI port is DVI-D (digital only) so it won't be able to run your
analog monitor. See this page for more on xrandr:

As to the crash, anyway you could get a proper backtrace with GDB?
