Since upgrading from the early 6.6.x series, I'm no longer able to
switch resolutions with Ctrl+Alt+(+/-).

I don't know whether it is still supposed to work these days. Given that we can very easily have different resolutions of multiple screens with the new driver, I don't know what would be supposed to happen when you hit Ctrl+Alt+(+/-).

At least, I guess you can switch resolution with xrandr --output LVDS --mode 1024x768 or so?

On a probably related note, if I start xawtv for the first time and
switch to fullscreen mode, it will switch to 640x480 (but uncentered).
After that, any attempt to launch xawtv will result in failure:

  This is xawtv-3.95.dfsg.1, running on Linux/i686 (2.6.21-2-k7)
  xinerama 0: 1024x768+0+0
X Error of failed request: XF86DGANoDirectVideoMode
Major opcode of failed request: 136 (XFree86-DGA)
Minor opcode of failed request: 1 (XF86DGAGetVideoLL)
Serial number of failed request: 69
Current serial number in output stream: 69

I get this the first time I start xawtv after booting :) I don't know whether it works with earlier versions here.
