Since the ABI change means that I can't use the previously patched server now (described in the bug), I tested the new mga driver. Situation now is a bit better than earlier: now switching to console mode and back (or restarting X) suffices - some code to turn on the 2nd head is apparently already there and is executed when console mode is restored, but not on initial set up of the video mode.
