I posted to the Toshiba Linux users mailing list (tlinux-users@linux.toshiba-dme.co.jp), and received confirmation from another user who is experiencing the exact same problem. He has a different model of Toshiba laptop and using qwerty layout, so this bug is not specific to the L5 or dvorak.
The Linux keyboard driver has always seemed to need special attention with regard to Toshiba laptops, so I suspect the problem is there.
