Shared memory (shm) in the 2.4 kernels is handled by a virtual
filesystem, so just like /proc, you need to mount it on boot by
putting something like this into /etc/fstab:
none /var/shm shm defaults 0 0

(Is /var/shm the right spot for this? What happens if one mounts
/var/shm and then /var?)

Of course, you also need to create the /var/shm mountpoint.
