I don't think so. You really shouldn't have any other includes
before the header. It isn't nice to silently drop broken pages
from the index, however. Perhaps it should display a nice big
warning on the index page when an issue is broken?

The Makefile dependencies are broken. I mailed a fix earlier
today, but it still doesn't capture that the annual indices should
depend on the issues for that year. I'll try to come up with a
solution this evening.
