Could you please provide the brief ITP skel (as in every other ITP)?
Here is the template:

    * Package name    : %(package)s
      Version         : x.y.z
      Upstream Author : Name <somebody@example.org>

    * License         : (GPL, LGPL, BSD, MIT/X, etc.)
