After talking to upstream, it turns out that gr4_buttons is to be considered a temporary solution. Apparently, the linuxwacom project (packages wacom-tools and xserver-xorg-input-wacom) has added support for the tablet's express keys and wheel, which makes gr4_buttons unnecessary.
