Your main problem seems to be about the entrails of the simple-cdd
made ISO. I fail to find a mailing list address via
  https://tracker.debian.org/pkg/simple-cdd

says that simple-cdd creates a "customized DebianInstaller CD".
So, given that debian-installer bug reports go to 
  debian-boot@lists.debian.org
i'd say it is one of the plausible choices.

Another one would be to file a bug report against simple-cdd and hope
for a helpful reply. Before that i would try with a current netinst ISO
whether it works for you with PXE.
