I thought that this is handled via ACLs.

like 
acl_check_rcpt:

        drop
                dnslist = sbl-xbl.spamhaus.org


and you can run your own local blacklist by keeping them in a file e.g.:
/etc/exim4/sender_reject.list
and adding to the previous acl under dnslist=...
                senders = /etc/exim4/sender_reject.list

You have to restart exim after that. But if you just edit the blacklist you
don't have to restart exim.
