Yes, for :fail: to be allowed in ~/.forward one needs the allow_fail=true option in the appropriate router, see http://www.exim.org/exim-html-current/doc/html/spec_html/ch22.html#SECID130

The same can be achieved by putting

user: :fail:

in /etc/aliases, but I'm not sure if the option is needed or not in that case too.
