I confirm this behaviour, I just found clamav-milter logging to
/var/log/clamav/clamav.log.1 ...

not even talking about clamav-milter turning debug on even when I have
"Debug false" in clamd.conf, not supporting the codumented "-x" option :(

it seems that clamav-milter does not have possibility to reopen log file
turn on LogSyslog in /etc/clamav/clamav.conf


... seems there are more bugs in clamav-milter to report...
