Odd.  I am running the same versions as you and I have never had an
lvm failure since setting it up.  IIRC that would be over 2 years.

I have all my lvm volumes using reiserfs.

I set this up a long time ago using the inforation provided by the LVM
Howto's available on the net as well the LVM Howto included in  the 
doc-linux-text package.
