Debian system admin change control process

I would like to setup and install a debian server making sure that all 
steps of installation and administration are recorded in a change 
control process.

Is there anyone who already admin's debian in this way?  Do you have 
template change control sheets that you would be willing to share?

Is there any documentation on this process, e.g.

Installing Debian:  Setting up lang, keyb, drivers, partitions, etc.  - 
should I record this manually, or is there a log of this process?
Managing Debian:  Performing updates, e.g. apt-get update - again, is 
there a log available?
Etc..
