If you mean writing your own initscript, a template (as documented in the
manpage) is available:
/etc/init.d/skeleton

Install 'bootlogd' and edit the file:
/etc/default/bootlogd so that the line:
BOOTLOGD_ENABLE=Yes
Then your startups are logged to /var/log/boot
