The make-kpkg stuff seems to work quite well, but don´t use the
append-to-version parameter, there are problems when trying to install
additional modules that don´t come with the kernel.

I've been usng 2.6 kernels for a while now, and the append-to-version
has not caused me any problems.


Otherwise, you´ll need the module-init-tools package and some other
tools at relatively recent versions. Have a look at the kernel
documentation for this, it´s in the Documentation/Changes file.
