I also have an ATI Radeon VE QY (rev. 0).  The main difference is that mine is 
on 01:00:00, and it has 64 MB of memory.

Your XF86Config-4 looks almost identical to mine, except that I don't specify 
the BusID or the VideoRam - XFree86 auto detects that just fine for me.  
However, it looks from your log like you might have two vide cards - a Mach64 
also?  so auto-detection might not pick up the right one.  If you specify one 
of these parameters wrong, that might be causing your problem.  I would at 
least remove the VideoRam line and let XFree86 auto-detect that.

My XFree86.0.log looks very similar to yours as well, except that it doesn't 
seg fault.  I have attached my log from just before yours seg faults through 
the end, though I don't know if it will be helpful.

My /usr/X11R6/lib/modules/drivers/radeon_drv.o is dated 2003-02-25, if that 
makes a difference to you.  From your log it looks like this module might be 
causing your problems.  It belongs to the xserver-xfree86 package.
